MY MISSION is to assist the elderly in maintaining their independence and remain in the community by offering individualized services designed to meet their specific physical, emotional, cognitive and financial needs.
DECIDING how best to care for an ailing parent, spouse or loved one can be both difficult and stressful. You may be asking yourself:
¨ Is a nursing home the best solution?
¨ What other options are available?
¨ How will needed care be financed?
¨ Who can assist with the day-to-day coordination and assure that a responsive, client-centered plan is put into place?

Arranging home care, transitional living or skilled nursing care
as needed.

Coordinating and monitoring services from the many health care
providers and other social and community resources required to ensure optimal
care. Intervening in the event of an emergency, breakdown in the services plan,
or other crisis.

Making referrals for any specialized services that may be needed
by your loved ones.
¨ Offer ongoing counseling and support throughout the entire care process.
PROFESSIONAL QUALIFICATIONS:
I hold a Masters Degree in Social Work with a Gerontology specialty and I am certified in both Social Work and Care Management. For the past 15 years, I have been providing services to the aging and their family members. This includes placement in adult day care programs and nursing homes, facilitation of support groups, and care management. I am a member of the National Association of Professional Geriatric Care Managers, National Association of Social Workers, and American Society of Aging.
